How do you collect Name on liebling subscribe button

I’m using the liebling template and I want to collect names. Do you know how to do one of the following:

Option 1

If you click on the subscribe button underneath the site title

You are led to a page that doesn’t have a name field. How do I add it?

Option 2

Or, how can I change the subscribe button here into a modal?
