I made some changes to posts but the modified date on the sitemap.xml next to those posts hasn’t changed.
Are you using any seo plugin for that purpose? If yes then which plugin?
Ghost has no plugins, so that’s probably not it. 
I do see my sitemap update regularly. I’m definitely seeing the changes I made yesterday on my sitemap today. How long ago were your changes?
sitemap.xml will typically be updated immediately, at least on Ghost’s side. Do you happen to have some other caching enabled in your hosting setup?
