I currently use a custom domain with a hosted Ghost account and I’d like to change my domain. Will Ghost route URLs to old blog posts that are out in the world and that use my old domain to the the appropriate new URL with my new domain?

If you’re currently using mydomain.com and want to switch to newdomain.com for your Ghost site, everything on your Ghost site will be updated but other links on other websites won’t be updated and will 404. Ghost only ever knows about your current domain.
If you want to switch domains, you’ll need to set up an external service to redirect from the old one to the new one (like Cloudflare). Keep in mind, as well, that any SEO you built up with the current domain will be impacted when switching to a new domain, so beyond the technical difficulties, you should also consider that angle, too.
