Is it possible to use a YouTube as the post feature? Has anyone changed a theme to do this? And if so what was your strategy?
Yes, the Eston theme, which I’ve highly customised for my blog, had this functionality built in. I say “had” since the theme was recently pulled from Themeforest.

Thanks @Stromfeldt for link, another theme customisation strategy would be to inject a bit of JSON with object containing instructions and then use jquery to swap in theme. Was hoping for something a little less clunky.
