The code injection is not loading in 404 pages. It’s quite important that the code injection loads in every page.

The premise on 404 pages is that they should be minimal. But, if you can customize your theme, you can make sure it loads. You need a page called error-404.hbs added in the root of the theme. That’ll override Ghost’s existing behavior.
