I recently upgraded to the latest ghost form 1.9.
when adding a bookmark it shows correctly in the admin panel but in the front end, it gets distorted and looks like this.

Howdy there! Your theme is missing the CSS to style the new bookmark card. We’ve got some default CSS on our docs that should help you with that:
Just add it to your theme styles
